I decided to give this book a chance but I must say I am disappointed.I don't like the ending. the book lacks depth. I can't make the link between the title and the actual storyline except for the end when they get married...

The narrative that emerges is a fine accomplishment, tying together the horrific stories of countless children against a merciless landscape of undersupplied refugee camps, belligerent authority figures and the constant threat of renewed violence. London tells of children forced into prostitution and military service, Burmese refugees unable to leave their dreary Bangkok apartments for fear of deportation and other tragic consequences of conflict; the stories are chilling and London is an able interviewer, getting children to open up by joining their soccer games and getting them to draw pictures. London began his project at age 21, and has a neophyte's penchant for self-regard and melodrama ("She is looking though her life, to some place else, some future bliss that is forever out of reach"); letting the stories speak for themselves would have bolstered their resonance. Insightful, important and very moving

I was looking for a book about the experiences of children during wartime and found this on Amazon. It was exactly what I was looking for. London's writing was empathetic, engaging and respectful of what the kids had to say. He was honest about what he learned and the preconceptions he was forced to abandon along the way. I liked that he went to war zones on three separate continents and reported stories from wars both well known (Bosnia) and not so well known (the Congo). Moving and insightful. Highly recommended.

Amazing -- full of emotion

I bought this book for a project in graduate school. Of all the books I read during the 2-year program, this was my favorite. It is amazingly touching and full of all sorts of emotions. The center section with pictures and drawings really makes the stories come alive. I would love to meet the author and talk about his work even more.

Just be aware, this is not a novel, nor would I recommend it for anyone with a weak stomach or anyone under 18 years old. The stories can be very intense!

Worth reading

A good basic read around some of the worlds most current conflicts (Burma, Bosnia, Congo), with some basic history that the world hasn't changed that much since roman times. I would recommend it as an introduction to how war affects children and their families and some of the history of what these conflicts have been based around.

It has made me want to improve my world histroy further as I'm not sure too many people have a grip on the actual vs what the general media feeds us.

Must-read for any manager or new employee

This is a really useful book filled with sage advice for anyone assuming a leadership position, particularly as a former outsider to the organization. It's filled with reminders that you don't walk in with "the answer," that instead winning the trust and respect of your cohorts is a learning process that you should begin with great intensity. The first half of the book relates directly to someone who is assuming a management role, the next quarter of the book is about what to do as a new employee serving under a boss or bosses (perhaps as a mid-level manager). The last bit of the book gives a brief introduction to strategic thinking and the book concludes with questions to ask yourself (and your family) in evaluating your transition. It is applicable to any firm, church, non-profit, and even (mostly) the government.

Here's a summary of the points I gleaned:
- Establish your integrity in first 30 days.
- Learn all you can about the organization, put on your "historian" hat.
- Don't suggest changes without examining what has been done previously.
- Silence is not accession.
- Meet with everyone in the organization to evaluate their expectations. Ask them what they think you should focus on.
- Ask same questions of all so no one treated different and you have a cross-section.
- Look for "early wins," low-hanging fruit of improvements you can make or other things to boost morale.

Dealing with your boss in the first 30 days:
- Be proactive, assume it's on your shoulders to build the relationship and get the support you need.
- Schedule meetings to discuss expectations, evaluations, and personal development.
- Figure out what would give your boss "early wins." Make his priorities your priorities.
- Be proactive in doing things that will allow your boss to hear from people he trusts that you're a good worker.
- Don't bring your boss bad news early, at least without bringing good news too.
- Don't assume he will change. He has a style, foibles, accept them and work around then and move on. You can learn a lot from a bad boss, and you will likely have many.
- Examine how others relate to your boss and how he responds.

Strategy
- Begin figuring out who you need to move off your team immediately, whose roles need to change, and who you need to evaluate further.
- Think strategically. After your first 90 days you should be able to present a plan that is actionable.
- Evaluate the vision of the organization, its values, and use SWOT analysis.

Ask yourself feedback questions every week.
- What isn't going well. Why? What can you change?
- What are you least happy about. What can you change about it?
- What meeting troubled you the most? ""
- What conflict needs to be most resolved? ""

Family also has to be considered. How is your new role and time commitment affecting your family? Was the move worth it?

Very useful tool for transitioning to a new job

I'm 6 weeks into my transition as a senior executive at a new org and I'm finding the process very helpful. From the time you accept your position to crafting a 30-60-90 day plan I highly recommend for all transitioning executives and new managers.

Generally good advice; wish some of it was more practical

I liked the focus on the first 90 days in a new managerial role: tips for building credibility quickly, and tips for embarking on fixing what needs fixing while preserving what needs preserving. I liked the idea of breaking the first 90 days into 30-day chunks, with subplans for each stage. I liked the STaRS model as a way to characterize the situation: Startups where the whole group is creating something new; Turnarounds where you've got a fixer-upper and they know it; Realignments where they need some course corrections but not everyone knows or agrees; and Sustaining success where they're doing well and need to maintain it and grow. It was a useful model for comparing the likely challenges, opportunities, and areas of focus. I liked the insight that the STaRS model can apply at multiple levels: from a single project to a divisional unit to the whole company.

One aspect of the STaRS model struck me as needing elaboration. Different STaRS situations were, in part, characterized by whether the people involved agreed on how well things were going and what direction to take. That seemed unrealistically oversimplified. There'll often be people who are on board and those who aren't. The question is who. Maybe senior management has a consensus, but your staff doesn't. Maybe your staff has a consenus, but it's not the same as management's, and then your key customers have yet another view. Sometimes you've got overall consensus in one group or another, but one or two key people within the group see the situation differently. With each of the four situations in the STaRS model, it should be important to know where you have agreement on status and directions, and where you don't.

The book had its weak spots. Sometimes, the advice was overly simplistic, to the point that it felt like padding. I should set realistic goals? I should ask good questions? No kidding, really? I wouldn't think that anyone in a new managerial role would consciously plan to set bad goals and ask bad questions. (Their goals and questions might turn out to be bad, but they didn't go into the situation wanting to do the job poorly.) Also, the author's advice sometimes strayed quite far from business. Talk to me about the different challenges facing startups and turnarounds, sure, but I don't think he's qualified to tell me how to maintain a work-life balance (Chapter 9).

I had hoped the checklists at the end of each chapter would offer practical steps toward creating and implementing a 90-day plan, including good tips on when to do various things. Instead, the checklist items were rather nebulous and non-directive. The "checklist" label was a misnomer, compared to what I normally expect from a checklist. With each new chapter, I spent less and less time reading the checklist.

The anecdotes that started each chapter didn't do anything for me. Every one of them fell into one of two forms: a) Jane Doe succeeded by doing what I'm about to tell you to do, or b) Jane Doe failed by not doing what I'm about to tell you to do. They felt like simplistic self-justification, not insightful explorations of the advice being offered.

I would have liked some summary comparisons, for later reference and reminder, of the different STaRS situations, and a summary structure for the elements of the 90-day plan. This is all stuff that was strewn throughout the book, but some reference summaries would have been helpful.

All in all, though, there was good advice in the book, especially in comparing different aspects of the STaRS model. The book is suitable for anyone in a new leadership role. I don't see why some people are saying this is for senior leadership only. A new front-line supervisor could benefit from a 90-day plan too.

The Kindle version worked pretty well. I saw that some older reviews of the Kindle version complained about badly formatted tables and so on. That appears to have been resolved. The tables and figures are intact. However, you can't resize them and they don't use your chosen text size, so you might find them hard to read. The table of contents was pretty good, covering the chapters and the front and end matter, but not the subsections within chapters.

A "must have" for all Emergency and Radiology departments.

An excellent and imminently useful text. As a radiologist, I was wary of an imaging text written by an Emergency physician. My concerns were unfounded. The radiologic information is extremely accurate and in depth. The material is presented in an easy to read and understandable manner with many references to ACR, ACEP and other appropriate clinical guidelines. The large number of well labeled and well explained images and diagrams are excellent and the book is organized in a manner that will make it useful as a daily reference. The section on basic principles of chest interpretation should be required reading for all clinicians who view chest radiographs. A "must have" text for all emergency and radiology departments.

Finishing my emergency medicine residency, I highly recommend Dr. Broder's book "Diagnostic Imaging for the Emergency Physician." From head to toe, the book systematically covers the relevant emergent conditions to consider, the imaging modalities to choose from, the advantages and disadvantages of each and the interpretation of both the images and the results. Additionally at the end of each chapter, important clinical questions that emergency medicine providers face routinely are answered. Questions such as "Does this patient going to CT require IV, Oral or no contrast?", "Is a plain film adequate to exclude this diagnosis?" and "What should I do with a negative study?" are addressed. In my opinion, this book would be useful to anyone involved in emergency medical care.

Amazingly thorough and up to date!

By far the best book on emergency medicine radiology that I have found so far. More up to date than some of the prior classics, but more thorough than the newer books like the one by Fox. Very extensive discussions and tons, tons, tons of images! Must have for the emergency medicine resident or physician.

This book provides an introduction to health interoperability and the main standards used. Health interoperability delivers health information where and when it is needed.  Everybody stands to gain from safer more soundly based decisions and less duplication, delays, waste and errors.  

The third edition of Principles of Health Interoperability includes a new part on FHIR (Fast Health Interoperability Resources), the most important new health interoperability standard for a generation. FHIR combines the best features of HL7’s v2, v3 and CDA while leveraging the latest web standards and a tight focus on implementability. FHIR can be implemented at a fraction of the price of existing alternatives and is well suited for use in mobile phone apps, cloud communications and EHRs.

The book is organised into four parts. The first part covers the principles of health interoperability, why it matters, why it is hard and why models are an important part of the solution.  The second part covers clinical terminology and SNOMED CT. The third part covers the main HL7 standards: v2, v3, CDA and IHE XDS.  The new fourth part covers FHIR and has been contributed by Grahame Grieve, the original FHIR chief.

The third edition of this book has been fully revised, reorganized and extended. It provides a clear, readable introduction to healthcare interoperability for the IT professional, student, clinician and healthcare manager. Interoperability between healthcare computer systems depends on the development, implementation and deployment of appropriate standards working together as a tightly specified language.

The five new chapters on Fast Health Interoperability Resources (FHIR) and its implementation in Principles of Health Interoperability: SNOMED CT, HL7 and FHIR, Third Edition cover the most important new healthcare interoperability standard for a generation. FHIR combines the best features of HL7 v2, v3 and CDA, and leverages the latest web standards. In addition, the authors discuss the core principles of healthcare interoperability, SNOMED CT and clinical terminology, HL7 and interchange formats.

About the Author

Tim Benson graduated from the University of Nottingham as a mechanical engineer. He was introduced to healthcare computing at the Charing Cross Hospital, London, where he evaluated the socio-economic benefits of medical computing systems. He founded one of the first GP computer suppliers (Abies Informatics Ltd). There, with James Read and David Markwell, he helped develop the Read Codes, which became the national standard for NHS primary care and one of the two sources of SNOMED CT.  Tim led the first European project team on open standards for health interoperability, which led to CEN/TC251 and collaboration with HL7, where he was a co-chair of the Education Committee for several years.  He has also developed a family of short generic patient-reported outcome measures with R-Outcomes Ltd (http://www.r-outcomes.com).

 

Grahame Grieve graduated from the University of Auckland as a biochemist, and worked as a clinical diagnostic scientist at St Vincent’s Hospital, Melbourne, before spending four years performing medical research in Diabetes, Lipid Metabolism, and Oxidation. He then switched focus, and joined Kestral Computing P/L, a Laboratory and Imaging Information Systems vendor, where he ended up as Chief Technology Officer, before leaving to establish his own consulting business, Health Intersections Ltd (http://www.healthintersections.com.au). A growing involvement in integration, and interoperability, lead him to the HL7 community where he has led committees and edited standards for HL7 v2, v3 and CDA. The outcome of this was the recognition that something new was needed, and this led to the creation of the FHIR specification, which now consumes his life.

This is third edition of Tim Benson's now standard text on healthcare interoperability. It does focus on the specific technologies listed in the title, but it's more than just a handbook. Instead, it begins with a careful discussion of interoperability, and why building interoperable solutions in the healthcare domain has proven so difficult. The analysis is insightful, and well worth reading for any healthcare professional, informatics, or developer. The book offers no magic bullets, but understanding the nature of the problem is key developing solutions.

In the second part of the book, Benson moves on to a surprisingly detailed discussion of SNOMED CT, first considering its history and guiding principles, and then delving into the details. It may seem odd that the title of the book only mentions SNOMED. Why not ICD-10, LOINC, or any of the other vocabularies that play such a basic role in EHR systems today? In fact, Benson does discuss ICD_10, LOINC and UMLS. But SNOMED CT is presented as an example of how a vocabulary (really, an ontology) ought to be designed, and is compared in some detail with position dependent coding schemes. In my opinion, this was useful as practical example of the theory discussed in the first part of the book. But it also made me think about software design.

The remaining sections of the book focus on HL7, XDS, and FHIR. Unfortunately, the treatment of HL7v2 was brief, almost too brief. But that's also inevitable. It's simply not possible for a book such as this one to cover the protocol in detail. Instead, the book covers enough detail to understand the essentials of message structure, encoding, and the event model. Examples include patient identification (PID) and visit (PV1) But the main focus here is providing a reference point.

The next chapters consider HL7 V3, including the Clinical Document Architecture (CDA) and information model (RIM) . The coverage of HL7 V3 was more detailed than I expected, including a discussion of constrained models and the dynamic model. But again, much of the focus was on what has proved successful in HL7 V3, what has not, and why. In this part of the book, as elsewhere, there is a focus on common themes, and we are able to see how different technologies are used to address the same problems.

At this point, I'll skip over the section on XDS and move on to FHIR. This shouldn't be taken as a criticism of the chapter on IHE XDS. Rather, I feel less qualified to comment here (and this review is already getting a bit long!)

The final part of the book, co-authored with Grahams Grieve, the creator of FHIR, presents Fast Healthcare Interoperability Resources (pronounced "fire") This is a technology based on the principles of REST and agile computing, and which incorporates many of the best ideas of earlier generations of HL7. This book does a good job of presenting the main ideas from scratch, and does not fall prey to th e temptation to leave out important topics such as extensions, conformance and profiles, and implementing FHIR. Of course, there is much about the details of the specification that is not covered, but the book does a good job of presenting the core principles and underlying concepts.

In the end, we see enough of these technologies to understand how they can be used to address problems of interoperability, and that is the point.
